At Outlive Nutrition we provide specialised NDIS dietitian services either face to face or via telehealth. Our experienced team of Medicare-recognised dietitians is committed to supporting NDIS participants in achieving their nutrition and health goals through personalised, evidence-based care.

We can support clients who are self or plan managed under the following support categories:

We charge the same hourly rate for NDIS and non-NDIS clients.

Have you been referred by your GP?


We accept all GP referrals (please note there may be limits to rebates based on plan type)


Medicare Safety Nets

Medicare Safety Nets exist to help to ease the cost of out-of-hospital care. If you require regular appointments, tests, or referrals, out-of-pocket expenses can quickly pile up. The Safety Net is there to ensure that when you most need care, you can still access it.


Here’s how it works:

  • Each year, you will likely spend a certain amount of money in out-of-pocket expenses that are not covered by Medicare.
  • Once you have spent a certain amount of money on out-of-pocket expenses in a single calendar year (January 1 – December 31), you reach the Safety Net Threshold.


Checking your Medicare Safety Net threshold

If your Medicare online account is linked to myGov, you can check your threshold amount there. You can also check via the Express Plus Medicare app, or by calling Medicare directly.

Do you provide a medicare rebate?

Yes, the Medicare rebate for medicare referrals is $60.30 (from July 2024)

5 steps to seeing a NDIS Dietitian


Step 1: Check your NDIS plan

Confirm you have funding under either 

  • Improved Health and Wellbeing
  • Capacity Building – Health and Wellbeing
  • Core Supports (for some nutrition-related supports)


Step 2: Complete the online referral form

To complete this form you will need:

  • Your NDIS participant number
  • Your goals
  • Your support co-ordinator’s contact details
  • Plan manager’s details (if plan-managed)
  • Any relevant medical history or referrals


Step 3: Contact Outlive Nutrition

  • Email us: hello@outlivenutrition.com.au
  • Let us know you’re an NDIS participant
  • Specify if you prefer in-clinic or telehealth appointments
  • Discuss any specific dietary needs or concerns


Step 4: Prepare for Your Appointment

  • Complete any pre-appointment forms sent by email
  • Write down foods and drinks you eat for 3 days
  • List your current medications and supplements
  • Note down your main nutrition concerns and goals


Step 5: Meet your Dietitian

  • This might be in your home, online or in our Brisbane clinic

DVA Cards Holders

The Department of Veterans' Affairs (DVA) provides funding for dietitian consultations under specific conditions.


What are dietetic services?

Dietitians can help you improve your health with diet and lifestyle planning.


Who can receive it?

You may be able to receive dietetic services if you have an assessed clinical need and a:

  • Veteran Gold Card; or
  • Veteran White Card and your treatment is for an accepted service-related condition


What you can receive?

If you are eligible for dietetic services your dietitian may:

  • assess your health and nutritional needs
  • help you make the best food choices for your wellbeing
  • create a dietary plan for you to follow


From 1 October 2019, referrals to this service will be under the Allied Health Services treatment cycle. This means that a referral will last for up to 12 sessions or 1 year, whichever ends first.


These treatment cycles place you at the centre of your care and the general practitioner (GP) as your care coordinator.


There are no limits to the number of treatment cycles you can have.

You can also have:

  • a separate treatment cycle for each allied health service you need; and
  • treatment cycles for different allied health services at the same time


How you access

1. Get a referral to a dietitian covered by the Medicare Benefits Scheme (MBS) from your:

  • general practitioner (GP)
  • medical specialist
  • treating doctor in hospital
  • hospital discharge planner
